CareCall is a personalised service that provides you with:
• A Personal Health Coach who is a registered nurse specially trained for this role.
Your Health Coach may provide you information, support and tools to make health decisions for you and your family.

Free educational materials sent to your home.
• Your Health Coach may send you videotapes and printed materials on conditions such as back pain, asthma, diabetes and heart problems.

• Resources and tools such as health related reminders to help you schedule medical tests and get the most out of clinical appointments to take control of your health.




Health Coaches DO NOT provide treatment advice or try to influence your decisions.
Health Coaches DO provide unbiased, evidence-based information and support, so that
working with your clinician you can make informed decisions that best meet your personal
circumstances.
