Q. What does CareCall offer me?
A. CareCall is a free service offering you a personal Health Coach in addition to the services you already receive from your Practice, to help you better understand and manage your health. It gives you relevant health information when you need it and support and tools to make health decisions for you and your family.
Q. Who will I be speaking to when I call a Health Coach?
A. Health Coaches are registered nurses, who receive special Health Coach training, and who can speak to you about your healthcare concerns and can help get answers to your health questions.
Q. Can a Health Coach help me talk with my GP?
A. Yes. This service is meant to provide you with information, guidance and support to strengthen your relationship with your GP and other clinicians. In some instances, your Health Coach may also contact one of the doctors at this practice to co-ordinate your care. |

Q. What are the hours I can call a Health Coach?
A. You can call a Health Coach at 0800 028 4207 between the hours of 7am and 11pm Monday to Friday, and 9am and 1pm on Saturday.
Q. Can anyone participate in the CareCall Service?
A. Anyone aged 18 and over who is registered at the Practices involved with the service.
Q. What will happen if I choose to sign up?
A. You get access to your own Personal Health Coach
immediately! CareCall is a joint service between Surrey
Primary Care Trust and our Practice. It is operated by
Health Dialog UK Limited, an organisation working with
us to improve our patients’ healthcare. To make it
possible to maximise the benefits of the service to you,
we will send selected information such as your name,
address and limited health information to CareCall to
enable us to work with them to identify the most effective
ways to improve your healthcare. Personal information
shared between our Practice and CareCall will not be
shared with anyone else and will be maintained in strict
accordance with data protection laws and only be used
for the purposes of CareCall. |
